can I build this with payload?

I want to know if this flow can be built with Payload or if someone has suggestions on how to create this.

I have some collections: users, activities, and visits [image with example in comment]. Right now I can create visits for those activities in Front, the visit is a collection with a relationship between users and activities.

For some visits we need some extra info from the user, I was thinking of creating a collection called forms that has the block field to be able to create different forms and add a relationship to the form in activities. Then in Front, pull those fields and create the form.

I don't know the right way to save this data.

Can I add these new and random fields directly to a collection? (maybe the user or another like the visit collection)

I think this can be done with the form plugin.
